1.2 Treatment Requirements

All discounted services must be performed at Fusion Dental Implant .

Final pricing is determined after a comprehensive examination and written treatment plan; online estimates may vary.

Payment plans and financing are separate from discount programs.

2 DISCOUNT CATEGORIES

3 PROMOTIONAL CODES & OFFERS
3.1 Code Usage

Codes are case-sensitive; expired or invalid codes will not be honored.

Codes are non-transferable and limited to one use per patient unless stated otherwise.

4 PAYMENT & BILLING
4.1 Payment Terms

Discounted pricing requires adherence to the agreed payment schedule.

Missed or late payments may void the discount.

Insurance claims, if applicable, will reflect discounted amounts.

4.2 Treatment Completion

Discounts apply only to completed treatments.

If treatment is cancelled or altered, fees revert to standard rates for services rendered.

5 LIMITATIONS & EXCLUSIONS

Emergency or unplanned procedures may be excluded.

Some specialized or third-party services are not eligible.

Discounts do not reduce insurance copayments or deductibles; the practice will not inflate claims.

6 PRACTICE RIGHTS & MODIFICATIONS

The practice may modify, suspend, or terminate any discount program at any time; changes take effect upon posting on the clinic website.

The practice may refuse or revoke a discount for any reason, including suspected fraud.

Clinical judgment overrides any promotional pricing; treatment decisions remain solely with the doctor.

7 DISPUTE RESOLUTION

Direct discount-related complaints to management in writing; responses are issued within 10 business days.

Liability is limited to the amount of the disputed discount.

All disputes are governed by the laws of CA and fall under local jurisdiction.

8 COMPLIANCE & PRIVACY
8.1 Regulatory Compliance

All promotional activity complies with applicable federal and state healthcare regulations, including the Anti-Kickback Statute and, where relevant, the Stark Law.

8.2 Privacy & Data Security

The price calculator collects only the minimum information needed to send a personalized estimate (full name and e-mail address). These data:

are used once to generate and deliver the estimate (and, with opt-in, related follow-ups);

are retained no longer than 30 days unless an appointment is booked or continued communication is requested;

do not include X-rays, photos, diagnoses, or other protected health information;

are never sold or shared with third parties, except secure service providers under confidentiality;

are transmitted via TLS-encrypted connections and stored on secure servers;

can be deleted on request by e-mailing eduardkarionov@gmail.com.

8.3 California-Specific Disclosures (B&P Code § 651)

When advertising a discount in California, all promotional materials must clearly state:

The regular, undiscounted fee for each service.

The exact dollar or percentage reduction.

The start and end dates of the offer.
